Hydration Status of Exclusively and Partially Breastfed Near-Term Newborns in the First Week of Life

Se hizo un estudio de cohorte prospectivo observacional intra-hospitalario para evaluar los efectos del tipo de alimentación (lactancia maternal exclusiva [LME] vs lactancia parcial [LP]) en el estado de hidratación de recién nacidos casi a termino en la primera semana de vida. Se reclutaron 205 bebes de 35 a 37 semanas de gestación (82 en el grupo de LME y 123 en el de LP). La incidencia general de perdida de peso significativa (≥ 10%) fue de 18% sin diferencia significativa entre los grupos de LME y LP (18.3% vs 17.9%, P = .94). La incidencia de hipernatremia (niveles de NA en el suero ≥ 150 meq/L) fue de 2.4% en LME y 5.7% en el grupo de LP (P = .32). Los factores que se asociaron significativamente a la perdida de peso en el total del cohorte fueron: tener una madre que tuvo anteriormente una experiencia negativa con la lactancia materna (OR ajustado = 15.5, 95% CI = 2.1-115.7), exposición a fototerapia (OR ajustado = 9.0, 95% CI = 2.5-31.8) y parto por cesárea (OR ajustado = 6.7, 95% CI = 2.3-19.7).
